What to expect:
The first appointment is an in-person consultation/diagnostic evaluation session in which we will discuss your concerns, treatment needs, and background information. This process is collaborative and our therapeutic relationship will be confidential. Please complete and return the required paperwork prior to your first session. Or, if you prefer, arrive 15 minutes early to allow time to complete paperwork in the waiting room.
Call the office Monday-Friday from 9AM to 5PM at (773)-348-0722 or email email@example.com to schedule your first appointment. Please cancel or re-schedule appointments within 48 hours to avoid being charged in full for the missed session.
The first session (consultation/diagnostic evaluation) is one hour, for a fee of $250. Subsequent individual therapy sessions are 50 minutes for a fee of $190, and couples sessions are 60 minutes for a fee of $240. There is a limited number of spaces reserved for sliding scale clients. If I am unable to offer you a sliding scale, I will provide you with referrals.
Jennifer Levy, LCPC, CST is a sole, private, out-of-network provider. Ms. Levy does not bill insurance. She does provide a monthly paid statement to submit to your insurance carrier for “out of network” reimbursement. Please check with your provider to clarify reimbursement and information needed for claims.
Credit, cash, and check are all acceptable forms of payment. You can also use an HSA card. Ivy Pay is the credit card platform used by Ms. Levy. You will not be charged a service fee for using your credit card. Payment is expected at the end of each session.
Ms. Levy is not available for after-hours emergencies and does not carry a pager. Please call 911 or go to your nearest emergency room in the event of any type of mental health emergency.
